Veterinary Consent Forms 

Under the Veterinary Surgeons Exemption Order (2015) a referral form is required to be completed by your vet prior to a Veterinary Physiotherapist treatment. This is in addition to Veterinary Surgeons Act1966, which details that animals should only be treated by someone qualified to do so.

This means that before any rehabilitation or injury treatment for horses and before any treatment for dogs I am required to gain consent from a vet.
